I fleshed it out, we needed to make the new defense agency medical corp, for a lot of reasons, including military reasons.
There is no viable medical insurance path through state capital from the swamps. Give the small states cash tell them to adjust and make their senators help out. Ten make the Emergency Medical Corp, a uniformed service (it already is). Further reduction finds the Swamp is best at direct action at the emergency room level. I leave the problem to others expert in sandbox.
